Student Physicals & Participation in Athletics
Students of Columbus Public Schools are both allowed and encouraged to participate in school-sponsored organizations, including activities and athletics.
Prior to participating in an athletic event (i.e.: practice, competition or off-season workout), students must complete a physical examination and submit a School Activities Association-approved physical form to CPS. This physical form should include the signature of the medical professional who conducted the physical exam. The proctoring medical professional should also sign all local and state forms where required to provide clearance for the student to participate.
The physical examination required of all freshman entering high school for the first time is not an acceptable examination for athletic participation.
Furthermore, students who participate in contact sports (including, but not limited to: Football, Volleyball, Softball, Basketball, Diving, Wrestling, Track, Soccer and Baseball) may be required to take a neurological exam prior to participation in CPS Activities, in accordance with state and federal legislation.
In order to participate in any athletic event sponsored by CPS, students must have permission and clearance from each of the following six individual entities:
1) Student Themselves
2) Studentís Parent(s) and/or Guardian(s)
3) Studentís Family Physician/Medical Professional
4) Studentís Activity Sponsor/Coach
5) Columbus Public Schools Administration
6) Columbus Public Schools Athletic Trainer
If a student does not have full clearance from each of the above six individual(s), they are not allowed to participate in any athletic activity until that clearance has been granted.
